You may order your repeat prescriptions in several ways:

Telephone: We are more than happy to take prescription requests over the phone but please AFTER 11.00am. Please advise if you want to collect from the surgery or the pharmacy. Facsimile: We can also take fax requests on 01202 672756. Please advise if you want to collect from the surgery or the pharmacy. Post: We can also post your scripts out to you but you will need to send us a supply of stamped addressed envelopes. On-line: To request a prescription on-line please complete the following form and press the "Request Prescription" button. Please allow 48 hours before collecting from the surgery or 72 hours before collecting from the pharmacy. Fields marked * are required items of information.
